When I move house do I take the Sky dish with me (as I paid for installation) or do I leave the dish? Do Sky come and take it down and re-install at a new address?

You can do what you like with the sky dish but if in the fixtures and fitting form (supplied by your solicitor) you say that the sale includes the dish make sure you leave it.

I don't know what Sky's change of address procedure is so I suggest you give them a call and see if there are savings to be had by taking your old dish with you.
